Men's Open League: Team Registration Form

PLEASE NOTE: $100.00 discount for registering and paying in full by September 15, 2009
for Session 1 and/or Session 2!

• Cost: Session 1 (5 Games) - Wednesday Nights
  $700.00 per team plus $20.00 ref fee per game
  $140.00 per game per team
  (only $14.00 per player per game based on a 10 player roster)
• Cost: Session 2 (10 Games)
  $1,500.00 per team plus $20.00 ref fee per game
  $150.00 per game per team
  (only $15.00 per player per game based on a 10 player roster)

•      Please note: $100.00 discount for Session II for signing up for both sessions.

• Number of Games: 5 games for Session 1 and 10 games for Session 2
  

• Schedule:

  • Men's Open games will be played on Wednesday  nights.

  • Session 1: Begins Wednesday November 18, 2009

  • Session 2: Begins Saturday January 2, 2010

  • Session 3: Begins Friday March 15, 2010

  • Game schedules will be posted on web site

• Length of Games: Two 28-minute halves

• Size of Field: 105' x 150'

• Number of Players: 6 v 6 (including Keeper)

• Uniforms: All teams must wear matching uniforms. Uniforms will not be provided.

• Standings: Standings will be posted on web site

• Additional Info:

  • Open to men ages 18 and older

• Registration Info:

  • Completed Registration Form and $200 deposit required to reserve a spot

  • All players MUST sign a Waiver & Consent Form
    

  • All fees must be paid in full before 1st game

  • Registration Deadline for Session 1 is November 13, 2009

  • Registration Deadline for Session 2 is December 20, 2009

  • Registration Deadline for Session 3 is March 13, 2010

Attention all teams!  The following must be completed 30 minutes prior to your first game:

• Submit completed Registration Form
• All players must submit a signed Waiver and Consent Form
  
• Registration Fee paid in full
